Collection of the above mentioned material can be done during business hours at 3225 Ambassador Caffery, Lafayette, LA 70506; However, faxes can be sent 24 hours/7 days a week. Citizens’ privacy and confidential information will be protected. The information collected will be transferred directly to the federal authority and will not be retained by Pittman Broadcasting Services LLC, KVOL-AM 1330, Ware Investigations LLC or Joel Henderson.
